I've read several interpretations of these lyrics, ande I'm sure it's inspired by some story, but I also tend to believe these lyrics were intentionally ambiguous for listeners to fill in the blanks for themselves. (In fact, I think that had alot to do with the popularity of the song.)

It does have an uncanny "prophetic" feel to it - in the way that Freddy later died, but he didn't contract HIV until long after this song was written and became a hit. I don't think that's a valid connection.
